Tadalafil (Cialis/Adcirca — PDE5 Inhibitor)
OtherFDA: FDA-APPROVED (ED: Cialis; PAH: Adcirca...

Overview

FDA-approved ED, BPH, pulmonary arterial hypertension (Adcirca). Longevity research: weekly low-dose tadalafil shows cardiovascular protection, reduced fibrosis, improved exercise capacity in aging models.

Mechanism of Action

Selective PDE5 (phosphodiesterase type 5) inhibitor. Prevents breakdown of cGMP → sustained smooth muscle relaxation → vasodilation. 36h half-life vs 4–6h sildenafil. Emerging longevity research: PDE5 inhibition reduces cardiac fibrosis, improves mitochondrial function, reduces inflammatory cytokines. Weekly low-dose cardiovascular research ongoing.
