Exemestane

Exemestane (Aromasin — Steroidal Aromatase Inactivator)
HormonalFDA: FDA-APPROVED (breast cancer in...WADA: PROHIBITED — Category S4

Overview

FDA-approved for breast cancer. Steroidal structure provides mild androgenic effects alongside estrogen suppression. Preferred by some clinicians for TRT management due to gentler estrogen floor.

Mechanism of Action

Steroidal irreversible aromatase inactivator (Type I). Binds permanently to aromatase active site — enzyme is permanently inactivated, recovery requires new enzyme synthesis (~72h). Androgenic properties (weak) differentiate from non-steroidal AIs. Does not suppress estrogen below physiologic floor as aggressively as anastrozole.
