Finasteride

Finasteride (Proscar/Propecia — 5-Alpha Reductase Inhibitor)
HormonalFDA: FDA-APPROVED (Propecia: hair loss...

Overview

FDA-approved hair loss (Propecia 1mg) and BPH (Proscar 5mg). Commonly used alongside TRT to manage DHT-related side effects. Post-finasteride syndrome: persistent sexual dysfunction reported in subset of users — ongoing research.

Mechanism of Action

Competitive inhibitor of 5-alpha reductase type II (and some type I). Blocks conversion of testosterone → dihydrotestosterone (DHT). Reduces serum DHT by ~70%, scalp DHT by ~60–70%. DHT is primary driver of androgenetic alopecia and benign prostatic hyperplasia. Relevant in peptide/TRT context: managing DHT elevation on testosterone therapy.
